Use this form when you would like to make any changes to your schedule including drops, adds, or withdrawals.  The form requires an advisor's signature. 
 
Be aware that there are final dates for adding, dropping, and withdrawing from classes.  Please see the Academic Calendar for specific dates.

Use this form when you would like to propose a special project for earning one additional semester hour of credit in one course in which you are already enrolled in during a given fall or spring semester. Summer courses are not eligible for this option. The form requires an instructor's signature and must be accompanied by a drop/add form.
 
Be aware that there is a final deadline for adding a course intensification.  Please see the Academic Calendar for the specific date.

Major Declaration

Alteration Approval to Major/Minor

This form is to be used when a department has allowed for any alterations to be made to major or minor requirements on a student's record.  This form must be completed, signed and submitted to the Office of the Registrar for any change to be applied to a student's degree requirements.
