Registration for the Spring 2021 academic semester will soon be open online through the traditional MyDU format. Click on the appropriate link below to download any of the forms needed for special circumstances.

 

IMPORTANT NOTE (04/09/2020): During the 2020 Advanced Registration periods, students required to gain authorization from their advisor/dean may do so via DU email rather than providing a signed form in person. Students can complete the appropriate fillable PDF form below, save it, and then email the request to their Advisor as an attachment. Requests must be sent using your Dominican email address. Advisors will then be required to forward the completed form via their Dominican email with a statement granting their approval to starsconnect@dom.edu. In specific circumstances, Student-Athletes will require email authorization from Mark White. International students will require authorization from Sue Ponremy. Please email starsconnect@dom.edu with any questions you may have regarding this process.

Use this form when you would like to make any changes to your schedule including drops, adds, or withdrawals.  The form must be completed and forwarded to your advisor using your Dominican email address.

This application must be filed at the time of registration. No independent study courses or directed study course will be authorized without a completed application. The form must be completed and forwarded to your advisor using your Dominican email address.

Use this form when you would like to propose a special project for earning one additional semester hour of credit in one course in which you are already enrolled in during a given fall or spring semester. Summer courses are not eligible for this option. The form requires an instructor's signature and must be accompanied by a drop/add form. The form must be completed and forwarded to your instructor using your Dominican email address. 
 
Be aware that there is a final deadline for adding a course intensification.  Please see the Academic Calendar for the specific date.

Major Declaration

Title Description
Declaration of Major forms are available through the Rosary College of Arts and Sciences Advising Office. The above link will take you to RCAS website for Declaration of Major.

Alteration Approval to Major/Minor

This form is to be used when a department has allowed for any alterations to be made to major or minor requirements on a student's record.  This form must be completed, signed and submitted to the Office of the Registrar for any change to be applied to a student's degree requirements.
