This is a mandatory time sheet that should be used for all placements. Students have the responsibility to track hours weekly and have Field Instructors initial hours completed on a weekly basis.

If you are interested in doing a placement where you are currently employed. Please find application here. You must be placed in a department different from the department which you are employed, under a different supervisor and with a different set of tasks and responsibilities. If you have questions, please contact the Field Office.

All field placements must be approved through the field placement office prior to students beginning their practicum. 
Failure to comply with this policy will result in possible illegitimate placement and, therefore, jeopardize the student’s
opportunity to continue in the program.
