This is a mandatory time sheet that should be used for all placements. Students have the responsibility to track hours weekly and have Field Instructors initial hours completed on a weekly basis.

If you are interested in doing a placement where you are currently employed. Please find application here. You must be placed in a department different from the department which you are employed, under a different supervisor and with a different set of tasks and responsibilities. If you have questions, please contact the Field Office.

For students who envision having difficulty completing hours in the 9month academic year, an extension is available. Please note there is an additional fee associated with the extension.
